As a Grief Counselor, you will provide counseling, education, and support to hospice patients' families, community members, schools, workplaces, and contracted facilities. You will play an important role in helping individuals of all ages process grief while connecting them with meaningful resources and support.
include: Provide individual grief counseling for children, adolescents, adults, and older adults. Develop and document individualized bereavement plans of care in collaboration with the interdisciplinary care team. Facilitate grief support groups and maintain program documentation. Assess client needs and connect individuals with appropriate community resources. Educate families, community organizations, schools, and workplaces on topics related to grief, loss, and bereavement. Participate in memorial services and other Grief Services events. Maintain timely, accurate documentation in accordance with department standards. Collaborate with Unity Hospice staff to ensure compassionate, coordinated care. Comply with all applicable policies, regulations, and required training.
Strong counseling and interpersonal skills. Excellent verbal and written communication. Ability to build trusting, compassionate relationships with clients of all ages. Strong organizational and time management skills with the ability to manage multiple priorities. Ability to maintain confidentiality and professional boundaries. Comfortable speaking to community groups and providing educational presentations. Proficiency with Microsoft Office (Word, Excel, and PowerPoint). Master's degree in Counseling or Social Work. Previous experience working in the field of grief, loss, and end-of-life preferred. Current Wisconsin licensure as one of the following: Licensed Professional Counselor (LPC) Licensed Professional Counselor in Training (LPC-IT) Licensed Clinical Social Worker (LCSW)
