RN Care Manager - Full-Time - Day Shift

Community Care is hiring a RN Care Manager to join our PACE Program team. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, being part of a mission driven organization and who shares our mission to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This is a full-time clinic based (on-site) opportunity at our Racine location.

$8,000 Sign On Bonus! RN Care Manager Job Responsibilities

  • Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed. Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services. Complete all required documentation and charting and assists in the development and maintenance of member records.

Requirements

Requires an Associate degree in Nursing and a valid Wisconsin Registered Nurse license. Candidates must have at least 2 years of recent experience working with older adults or persons with disabilities.
