The Registered Nurse (RN) is a key member of the Assertive Community Treatment (ACT) / ACT Plus multidisciplinary team and is responsible for providing comprehensive nursing care and clinical support to individuals with severe and persistent mental illness, co-occurring disorders, and complex healthcare needs. The RN delivers community-based nursing services that support recovery, rehabilitation, health maintenance, medication management, and independent living.

Crisis intervention & care coordination
Coordinates emergency and crisis intervention services when necessary.
Assesses urgent clinical situations and initiates appropriate interventions.
Participates in crisis prevention and response planning.
Coordinates medical and behavioral healthcare services.
Ensures follow-through on medical appointments, treatments, and provider recommendations.
Communicates treatment changes and significant clinical information to team members and providers.

Candidates must be a graduate of an accredited School of Nursing with a current unrestricted RN license in Delaware and CPR certification. A Bachelor of Science in Nursing (BSN) and experience in behavioral health or community mental health are preferred.
