About the role

The Physical Therapist is responsible for evaluating patient strength and mobility to develop and implement customized treatment plans. They must also manage Medicare documentation and collaborate with care teams to improve patient functional outcomes.

Physical Therapist (PT) – Long-Term Care LP Therapy LP Therapy is seeking a licensed Physical Therapist (PT) to provide skilled therapy services in a Long-Term Care setting focused on restoring mobility, strength, and function.

Responsibilities

review physician referrals and medical records; evaluate strength, mobility, and functional ability; develop and modify treatment plans; administer therapeutic exercises and modalities; educate patients and families; complete timely Medicare documentation; collaborate with care teams; supervise PTAs and aides as required; support quality outcomes and productivity standards.

Qualifications

Bachelor's degree in Physical Therapy; current state PT license; LTC experience preferred; strong knowledge of Medicare guidelines; effective communication and teamwork skills. Work Environment: LTC facility; physical demands include lifting and mobility assistance.

Requirements

Candidates must hold a Bachelor's degree in Physical Therapy and a current state PT license. Experience in long-term care and a strong understanding of Medicare guidelines are preferred.
