The Registered Nurse serves as a member of the PACE Interdisciplinary Team to plan and implement care plans for frail elders. Responsibilities include conducting assessments, monitoring clinic care, and coordinating with the team to ensure participants can safely remain in the community.

Serves as a member of the PACE Interdisciplinary Team.
Participates as a member of the Interdisciplinary Team in the planning, implementation and evaluation of care plans that meet the objectives, standards and policies of the PACE model of care.
Demonstrates proficiency in administering nursing care in a professional and respectful manner with the goal of assisting frail elders to remain living in the community for as long as they can do so safely.
This position is full time M-F 8am to 4pm with no weekend or holiday hours.
Spanish speaking preferred.
This is a full-time position, Monday- Friday 8-4 position; no nights, weekends or holidays!

Candidates must be a Registered Nurse licensed in Massachusetts with at least two years of experience working with geriatric populations. Proficiency in Microsoft Office and the ability to travel locally are also required.
