Your Impactful Role as a Per Diem PTA
Provide skilled physical therapy services in patients' homes by implementing treatment plans established by the supervising Physical Therapist, helping patients improve mobility, strength, balance, and independence.
Monitor patient progress and document care accurately and promptly, communicating changes in condition and collaborating with the interdisciplinary care team to ensure high-quality patient outcomes.
Educate patients on home exercise programs, safe mobility techniques, fall prevention, and the proper use of assistive devices to support continued recovery between visits.
Must hold a valid Minnesota PTA license and CPR/BLS certification. Previous experience in home health or outpatient settings is strongly preferred, along with the ability to travel within the Twin Cities Metro area.
