About the role

Responsible for delivering resident care through planning, implementation, and evaluation while guiding ancillary personnel. Duties include medication administration, resident monitoring, and assuming the charge nurse role when designated.

Position Summary

Responsible for the delivery of resident care through the nursing process of planning, implementation, and evaluation. Guides resident teaching and the activities of ancillary personnel while maintaining standards of professional nursing. Understands and adheres to GCMC’s Compliance Standards as they appear in GCMC’s Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy. Keeps abreast of all pertinent federal, state and GCMC regulations, laws, and policies as they presently exist and as they change or are modified.

Education, License & Cert

  • Graduate of a licensed practical nursing program with a certificate in nursing
  • Current NYS registration/license as a Licensed Practical Nurse required
  • Current BLS‐CPR Certification required

Experience

• Entry level graduate of licensed practical nursing program and licensed or a permit in New York State.

Essential Functions

1.

Provides safe and accurate nursing care and other therapeutic measures according to policy and procedures.

1.

Able to explain rationale for actions performed.

2.

Provides resident education to the resident and/or family/significant other as indicated.

3.

Assists in direct resident care as necessary.

4.

Assists the RN with the admission process of residents in a safe and accurate manner.

5.

Assumes responsibility for the charge nurse role when designated by: 1.

Provides direction and leadership in resident care to nursing staff through delegation of assignments and ensuring that quality of care is delivered.

2.

Completes assigned tasks and duties for maximum effectiveness and functioning of the unit.

3.

Properly verifies and transcribes physician orders.

4.

Ensures that all orders are faxed to provider pharmacy.

5.

Facilitates the appropriate documentation for daily activities on the unit as well as for admissions and discharges.

2.

Resident Monitoring ‐ Monitors residents as appropriate for condition, status and safety according to nursing policy.

1.

Recognizes change in resident condition and reports it to the Supervising RN.

2.

Works to ensure resident safety is maintained in all aspects of care by following policies.

3.

Notifies the physician and family in a timely manner of changes in resident condition.

4.

Maintains continuity of care by contributing to resident status reports ie.

Participating in shift report.

5.

Documents on all resident care activities in an accurate and timely manner.

3.

Medication Administration‐Performs safe administration and complete documentation of medications in accordance with nursing policy. a) Follows medication administration policy.

4.

Safe Discharge/Discharge Planning 1.

Completes resident education as necessary and documents all education provided.

2.

Provides discharge instructions and gives accurate information.

5.

Demonstrates the ability to safely and effectively use department equipment.

1.

Demonstrates knowledge and ability to safely and effectively use equipment.

2.

Uses all equipment per policy (ie. lifts).

6.

Demonstrates working knowledge and understanding of emergency preparedness plans and operations within the nursing facility.

1.

Aware of and actively participates in emergency preparedness procedures and operations.

2.

Actively participates in all actual Codes and drills.

7.

Infection Control‐Provide safe and effective resident care in accordance with Infection Control policies and procedures 1.

Wears appropriate PPE as warranted.

2.

Follows hand hygiene protocol.

3.

Follows all precaution procedures.

Pay for this position is contract controlled at a rate of $38.20/hr.

Requirements

Requires graduation from a licensed practical nursing program and a current NYS LPN license. Current BLS-CPR certification is also mandatory.
