About the role

Provide spiritual support, guidance, and counseling to hospice patients, families, and caregivers facing end-of-life challenges. Collaborate with an interdisciplinary team to integrate spiritual care into individualized patient care plans.

Description The Hospice Chaplain will provide spiritual support and guidance to patients, families, and caregivers facing end-of-life challenges. The chaplain will play a key role in addressing the emotional and spiritual needs of patients, fostering a sense of peace and comfort during this critical time.

Schedule: PRN, Weekdays, Potential Weekends, On-Call as needed, Day Shift

Primary Responsibilities and Duties

Provide spiritual care and support to patients, families, and staff in a hospice setting, regardless of religious affiliation. Conduct spiritual assessments to identify the individual needs and preferences of patients and families. Offer counseling, prayer, and support during difficult times, including end-of-life discussions. Collaborate with the interdisciplinary team to ensure a holistic approach to patient care, integrating spiritual care into individualized care plans. Facilitate memorial services, bereavement support groups, and other spiritual gatherings as needed. Educate staff about the spiritual aspects of care and how to support patients' and families’ spiritual needs. Maintain accurate and confidential documentation of spiritual care services provided. Be available for on-call services and emergency support as required. Stay informed about current trends in hospice care and spiritual care practices.

Requirements

Job Qualifications: Bachelor's Degree or equivalent in combination with experience and training. Hospice experience preferred but not required. Board certification from a recognized body (e.g.; Association of Professional Chaplains) preferred. Previous experience in hospice, palliative care, or a healthcare setting is a plus. Strong understanding of community resources and social service programs. Strong interpersonal, communication and counseling skills. Ability to provide compassionate care and support to individuals from diverse backgrounds and beliefs. Strong understanding of ethical issues related to end-of-life care. Ability to build rapport with patients through a compassionate and caring demeanor. Physical Requirements: Capability to perform physical tasks such as bending, standing, walking, and climbing stairs for extended periods. Strength to handle medical equipment and assist patients with mobility and physical activities, when needed. Visual acuity and dexterity for performing tasks such as reading medical records, documentation needed to support patients, families and caregivers, etc. Ability to respond swiftly and efficiently to emergency situations. Capacity to work in various home environments which may include navigating stairs, tight spaces, or uneven surfaces. Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager. Job requires accepting criticism and dealing calmly and effectively with high-stress situations.
