Under operational direction of the performs the role of social worker as a member of the interdisciplinary team. The Hospice Social Worker completes psychosocial assessments and provides ongoing psychosocial support to patients and families.

• Qualifications include but are not limited to: Master's degree (MSW) from a School of Social Work accredited by the Council on Social Work Education Minimum one (1) year of Medical Social Work experience required; Palliative, Hospice and or Home Health Care preferred Electronic medical record experience preferred Ability to learn and assimilate the EHR Computer Program/System Demonstrated ability to work with health team personnel Bilingual (English/Spanish) a plus but not required Must have a valid drivers license and a willingness to use personal vehicle in the course of employment Must show proof of automobile insurance, proof of current auto registration and acceptable driving record (no more than 3 points within a 36 month period) Ability to meet the physical demands of the job (ie, Push, Pull, Lift/Carry 10 lbs) If you are a person with the dedication, the compassion, the qualifications, and the skills required...
