QualificationsQualifications Active RN License required Hospice nursing experience preferred Experience with Homecare Homebase (HCHB) preferred Strong assessment, communication, and care coordination skills Schedule & Work-Life Balance Monday–Friday, 8:00 AM – 4:30 PM On-call opportunities available with additional compensation Some locations offer dedicated on-call teams, resulting in backup coverage only Key Responsibilities: Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care. Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB). Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care. Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes. Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
An active RN license is required, with a preference for experience in hospice nursing and Homecare Homebase (HCHB) software. Candidates should possess strong assessment and communication skills.
