The primary purpose of this role is to support the Family Medicine Physician Practice and provide necessary coverage when other providers are unavailable. Key duties include diagnosing and treating acute and chronic illnesses, ordering and interpreting tests, prescribing treatments, and managing chronic medical conditions.

PERFORMANCE REQUIREMENT / DUTIES AND RESPONSIBILITIES Diagnose and Treat acute and chronic Illness Perform age-appropriate history and physical Order and interpret diagnostic and therapeutic tests relative to patient’s age-specific needs Prescribe appropriate pharmacologic and non-pharmacologic treatments to patient s age specific need Monitor the effectiveness of treatments Provide education of acute and chronic medical conditions Managing Chronic Medical Conditions Reconciling medication list Facilitating appointments to specialist Providing thorough exams and evaluations of patient’s health status Facilitate appropriate chronic care management (i.e. Diabetic eye exams, foot exams and nutritional counseling) Preventive Medicine Keeping track of and referring patient to appropriate preventive medical screenings (e.g. Mammograms and Colonoscopies) Counseling and reinforcing patients in healthy life style habits

Candidates must be a Primary Care Nurse Practitioner certified in Massachusetts and possess three years of experience in a Primary Care setting. Proficiency in EMR systems, accurate documentation, and good communication skills are required, while a Master's Degree, ACLS, and PALS are preferred.
