About the role

Provide compassionate end-of-life care and support to patients and their families during vulnerable moments. Serve as the eyes and ears of the interdisciplinary team by responding to urgent physical and psychosocial needs.

CNAs are privileged to be invited into deeply vulnerable moments with patients and families. Our team works closely together to ensure the highest quality experience, with CNAs serving as the eyes and ears of the interdisciplinary team. Quick response CNAs play a crucial role in our Even More Care program, responding to urgent physical or psychosocial needs and sitting beside as a patient is in the active stages of dying. CNA Qualifications

High school diploma or G.E.D. required. Must be certified by the state as a nurse’s aide with an unrestricted, active aide certification. 3 to 6 months of current experience as a Hospice Aide or Nurse’s Aide required. Hospice or Home Health Agency experience preferred. Must possess interpersonal skills sufficient to interact effectively with patients and their families, as well as all interdisciplinary caregivers, peers, subordinates, and supervisors. Must exhibit or demonstrate the ability to learn computer skills related to required clinical software and electronic clinical record utilization. Must have personal transportation, valid driver’s license, required automobile insurance, and the ability to safely and independently operate said vehicle.

  • PM to 8:30 AM The office is located in Plymouth Meeting, PA and sees patients within a 50-mile radius. Be part of a team willing to grow, listen, be heard, and be challenged. Health, Dental, Vision, 401k, PTO. Set the industry bar for the standard of care. Continuing Education Reimbursement. Ability to grow into a variety of different roles inside our team and organization. Competitive industry pay.

Requirements

Must have a high school diploma or GED and a valid, unrestricted state certification as a nurse's aide. Requires 3 to 6 months of experience as a Hospice or Nurse's Aide and a valid driver's license with personal transportation.
