  • Summary: The Hospice Admissions Nurse is responsible for providing the consistency and predictability necessary to transition a patient’s care between care settings, or levels of care in a timely fashion. The Admissions Nurse will position themselves as a clinical resource, representing the benefits that Valir Hospice provides in the market to a limited number of assigned accounts. The role may include explaining benefits, assessing eligibility, completing part, or all the admission process, and providing daily GIP visit services. They will partner with the sales team to develop and maintain referral source relationships.

  • Experience: Current Registered Nurse license. Current CPR Certification. 1-2 years of experience in home health or hospice. LTCF experience preferred. Valid Driver's License and proof of liability insurance coverage

Requirements

Candidates must hold a current Registered Nurse license, CPR certification, and a valid driver's license with liability insurance. One to two years of experience in home health or hospice is required, with long-term care facility experience preferred.
