The qualified Licensed Practical Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Boost Home Healthcare policy.
Implement the nursing care plan for each patient.
Provide nursing services, treatments and diagnostic and preventative procedures as assigned.
Initiate preventive and rehabilitative nursing procedures as appropriate.
Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
Maintain accurate and complete records of observations, treatments, and care of patients.

Graduate of an approved school of practical (vocational) nursing.
Licensed practical (vocational) nurse in the state of job posting.
Minimum of one (1) year as an LVN in the Home Health Care Setting.
Must have current BLS Certification.
Candidates must be a graduate of an approved practical nursing school and hold a valid LPN license in the state of employment. A minimum of one year of experience in home healthcare and a current BLS certification are required.
