About the role

The Occupational Therapist will evaluate patients in a long-term care setting to develop and implement individualized treatment plans. Key duties include providing ADL training, managing adaptive equipment, and documenting progress according to Medicare guidelines.

Occupational Therapist (OT) - Long-Term Care LP Therapy LP Therapy is seeking a licensed Occupational Therapist (OT) to provide evaluation and treatment services in a Long-Term Care setting, helping patients regain independence and improve daily functioning.

Responsibilities

review medical records; evaluate patient abilities and needs; develop and implement individualized treatment plans; provide ADL training, adaptive equipment, and environmental modifications; document progress per Medicare guidelines; collaborate with interdisciplinary teams; supervise COTAs as assigned; support patient satisfaction and quality outcomes.

Qualifications

Bachelor's degree in Occupational Therapy; current state OT license; LTC experience preferred; strong knowledge of Medicare documentation; excellent communication and organizational skills. Work Environment: LTC facility; physical demands consistent with therapy services.

Requirements

Candidates must hold a Bachelor's degree in Occupational Therapy and a current state OT license. Experience in long-term care and strong knowledge of Medicare documentation are preferred.
