About the role

The Pharmacist is responsible for verifying medication orders, dispensing drugs, and monitoring therapy to optimize patient outcomes in an inpatient rehab setting. They collaborate with an interdisciplinary team to provide clinical support and ensure regulatory compliance.

Description Exalt Health, an acute rehabilitation hospital, provides an intensive rehabilitation program and admitted patients must be able to tolerate three hours of intense rehabilitation services per day. Focused on caring for patients with complex rehabilitative needs such as stroke, spinal cord injury, brain injury, head trauma, medically debilitation conditions, neurological disorders, cardio-pulmonary amputations, orthopedic injuries, and multiple major traumas.

The Pharmacist will play a vital role in our inpatient rehabilitation hospital, ensuring the safe and effective use of medications to optimize patient outcomes. This position involves collaborating with our interdisciplinary team of healthcare professionals to provide comprehensive pharmaceutical care, patient education, and drug therapy monitoring.

  • Review and verify medication orders for accuracy, appropriateness, potential drug interactions, and allergies.

Requirements

Licenses or Certifications Current, unencumbered license to practice as a Pharmacist in the state where currently practicing.

Education, Training, and Experience B.S. or PharmD from an ACPE-accredited School of Pharmacy
