Seeking a compassionate and professional PRN Nurse RN to conduct home visits and provide high-quality, individualized care to clients in their residences. Conduct in-home patient assessments, including health history, physical exams, and evaluations of the home environment
Monitor patient conditions, vital signs, and report changes to the supervising nurse or agency
Assess health status and develop individualized, comprehensive care plans tailored to client needs
Document care provided and health status updates with accuracy and efficiency
Educate patients and families on disease prevention, medication management, and overall health maintenance
Collaborate with the care team to ensure coordinated and effective service
Provide care with empathy, respect, and strong communication skills
Requirements: Current and valid RN or LPN license in the state of South Carolina
Minimum of 1 year of clinical nursing experience (home care preferred)
CPR certification (BLS)
Reliable transportation and valid driver’s license
Strong time management and independent decision-making skills
Ability to travel within the designated service area
Work Details: PRN (as-needed) availability required
Home visits required; each visit lasts approximately 30 minutes
Minimal documentation required following each visit
Must be able to travel independently to client homes within the service area
