About the role

The Clinical Social Worker provides behavioral health triage, assessment, and crisis intervention for patients in the Emergency Department. They collaborate with an interdisciplinary team to facilitate appropriate discharge planning and referrals to the correct level of care.

Job Summary

The Emergency Department Clinical Social Worker is responsible for the provision of behavioral health services including triage, assessment, intervention, and discharge planning for patients with behavioral health needs in the Emergency Department. The Clinical Social Worker functions as a member of the interdisciplinary care team providing behavioral health and social work expertise.

Duties & Responsibilities

  • 1.
  • The Emergency Department Clinical Social Worker is responsible for the emergency clinical assessment of patients, conveying clinical findings and recommendations to the attending physician, and facilitating transfer or discharge to the appropriate level of care.
  • 2.
  • Maintains all required documentation, following all departmental procedures.
  • 3.
  • Provides psychosocial assessment, crisis intervention, and brief counseling for patient presenting to the Emergency Department with a behavioral health or psychosocial need.
  • 4.
  • Coordinates appropriate referrals and facilities mandating reporting when a safety issue is identified.
  • Utilizes clinical supervision as needed for complex cases.
  • 5.
  • Collaborates and coordinates care with multidisciplinary team and actively provides feedback and recommendations to patient care.
  • 6.
  • Understands and accurately assesses the appropriate level of care determination.
  • 7.
  • Facilitates an appropriate disposition plan for patients demonstrating knowledge of resources including availability, eligibility, cost factors, and types of service.
  • 8.
  • Observes relevant hospital and department policies and procedures including risk management, infection control, and safety.
  • 9.
  • Maintains appropriate licensure, when applicable and enhances professional growth and development through continuing education.
  • 10.
  • Participates in supervision with Clinical Supervisor, either in individual or group setting, as scheduled by Supervisor.
  • 11.
  • Performs all other job related duties as assigned.

Requirements

A Master’s Degree in Social Work or related field. Licensure preferred. Experience in behavioral health required. Experience in a hospital setting strongly preferred.
