The Providence Center Case Manager Pawtucket Health Home Team coordinates services for consumers to minimize stressors and to facilitate the reduction of psychiatric and substance abuse disorder symptoms.

The Case Manager supports clients in reducing psychiatric and substance use disorder symptoms, managing stressors, and achieving the highest possible level of independence and community functioning.
They deliver person-centered, recovery-oriented case management as part of a multidisciplinary team, providing in-person and community-based support that aligns with program standards.
Responsibilities include conducting comprehensive assessments, developing individualized care plans, and connecting clients to housing, healthcare, education, employment, and other community resources.
The role also involves ongoing monitoring, crisis intervention, and close collaboration with the client care team and partner agencies to promote recovery while ensuring adherence to all applicable standards and regulations.
Human Services or related. Experience in Human Services field preferred. Must have valid driver's license, registration, current inspection sticker and insured auto. Strong knowledge of community resources and systems, including housing, education, employment, and healthcare. Excellent organizational, problem-solving, and time management skills. Ability to build rapport and establish trust with diverse clients and community partners. Cultural competence and commitment to providing equitable and inclusive services. Ability to communicate effectively, both verbally and in writing, to convey information clearly and collaborate with multidisciplinary teams. Proficiency in documentation and electronic record-keeping systems, including ability to document in electronic medical records. Ability to travel within the community with reliable transportation. Bilingual preferred. For positions designated as bilingual (Bil), proficiency in the required language is mandatory, and the language assessment must be passed no later than 30 days of starting the role.
