The Community Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hospice Home Care team. In this rewarding role, you will provide compassionate end-of-life care and support to patients and families in the comfort of their homes.

Current New York State Licensed Practical Nurse (LPN) license.
1 Year LPN experience
1 or 2 years of homecare and/or Hospice experience preferred
Valid driver's license and reliable transportation.
Strong communication, documentation, and organizational skills.
No hospice experience? No problem. We welcome nurses who are passionate about caring for others and are committed to learning.
We are happy to arrange a ride-along opportunity with one of our experienced preceptors so you can learn more about hospice nursing firsthand.
$24.00-$31.10 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

Must hold a current New York State LPN license and possess a valid driver's license with reliable transportation. At least one year of LPN experience is required, with homecare or hospice experience preferred.
