About the role

Conduct new admission evaluations and manage the treatment and discharge review of patients. Coordinate with interdisciplinary teams and communicate progress and goals to patients' families.

Windsor Home Health is looking for a home health Physical Therapist on a Part-time basis and/or PRN.

New admissions evaluations.

Review of potential discharges.

Staff training and/or communication of patient progress.
