About the role

The Nurse Practitioner provides compassionate nursing care to terminally ill patients and support to their families during bereavement. They are responsible for conducting patient encounters to determine initial and continued eligibility for hospice services.

Hospice Nurse Practitioner Full-Time

Parker Jewish Institute Comprehensive Community Hospice is seeking dedicated and compassionate full-time Nurse Practitioner to join our team. We want to speak with naturally caring, empathetic NPs who are focused on providing the best quality of end-of- life care possible for every patient. The Hospice Nurse Practitioner will be able to provide nursing care to the terminally ill Hospice patient, provide assistance and understanding to the family in the home care situation and in time of bereavement, and work as a member of the Hospice team in providing Hospice care.

Community and Inpatient Hospice at Parker Jewish Institute provides comfort, relief, and peace of mind to individuals with advanced or life-limiting illness and their families. We are an intimate and unique not-for-profit program that provides personalized care to ensure the comfort, enhance the quality of life, and preserve the dignity of our patients. Your role will be to ensure these needs are met by conducting encounters with our patients to determine their initial and continued eligibility for hospice services.

About Parker The Parker Jewish Institute for Health Care and Rehabilitation, conveniently located on the Queens-Nassau County border in New Hyde Park, New York, is a non-profit health care facility that offers inpatient programs such as sub-acute/short term rehabilitation, long-term care and nursing home care, as well as community health services encompassing certified home health care and a comprehensive community hospice program that serves terminally ill patients in their own homes or in nursing facilities, including Parker's nursing home. Position Qualifications Current New York State license as a Nurse Practitioner (NP) Degree from an accredited NP program 1+ years of experience as an NP Previous experience in hospice or home care a plus Able to use computer technology and electronic medical records in the field Access to reliable transportation and willing to conduct home visits

Requirements

Candidates must hold a current New York State Nurse Practitioner license and a degree from an accredited NP program. At least one year of experience as an NP is required, with a preference for those with hospice or home care experience.
