The Nurse Practitioner provides compassionate nursing care to terminally ill patients and support to their families during bereavement. They are responsible for conducting patient encounters to determine initial and continued eligibility for hospice services.
Parker Jewish Institute Comprehensive Community Hospice is seeking dedicated and compassionate full-time Nurse Practitioner to join our team. We want to speak with naturally caring, empathetic NPs who are focused on providing the best quality of end-of- life care possible for every patient. The Hospice Nurse Practitioner will be able to provide nursing care to the terminally ill Hospice patient, provide assistance and understanding to the family in the home care situation and in time of bereavement, and work as a member of the Hospice team in providing Hospice care.

Position Qualifications Current New York State license as a Nurse Practitioner (NP) Degree from an accredited NP program 1+ years of experience as an NP Previous experience in hospice or home care a plus Able to use computer technology and electronic medical records in the field Access to reliable transportation and willing to conduct home visits
