Position Overview Reporting to the Hospice Administrator, the Hospice Social Worker delivers appropriate social work services to assigned patients, residents, family members and significant others. The Social Worker is responsible for performing initial psychosocial, emotional, spiritual and bereavement assessments, ongoing assessments, and assists in the development and implementation of goal-directed IDG care plan while serving as an advocate for the patient and family members.
