The Breast Health Nurse Navigator assesses and manages barriers to care for patients with BiRad 4 and 5 diagnostic mammograms. They coordinate services within the Breast Center and community resources while collaborating with Oncology Nurse Navigators.
The primary purpose of the Breast Health Nurse Navigator position is to assess and manage barriers to improve timeliness of care, quality of care, clinical outcomes, and patient experience.
Responsible for providing individualized assistance to patients, families, and caregivers as well as internal and external staff and providers.
The Breast Health Nurse Navigator serves as a central point of contact for identified patients confronted with BiRad 4 and BiRad 5 diagnostic mammograms.
Arranges services within the Breast Center and resources available through the community or service agencies, providing education regarding follow up needed.
The Breast Health Nurse Navigator will work collaboratively to coordinate patient care needs by communicating with the Oncology Nurse Navigator should the need for services of the Cancer Center.
Requires an active Mississippi RN license and at least 2 years of nursing experience. A Bachelor's degree in Nursing and experience as a breast nurse navigator are preferred.
