Provide compassionate, home-based care to hospice patients with a prognosis of six months or less to live.
Collaborate with an interdisciplinary team (physician, social worker, chaplain, CNA, HHA, volunteers) to develop and execute individualized care plans.
Conduct thorough patient assessments, monitor vitals, evaluate breathing, and manage pain using prescribed medications and treatments.
Ensure the availability of necessary medical equipment, oxygen, and medications.
Educate patients and families about care plans, medications, hospice goals, and what to expect.
Offer holistic care that addresses the physical, emotional, and spiritual needs of patients and their families.

Requires an active, unrestricted RN license in Michigan and at least one year of nursing experience. Must have reliable transportation, a valid driver's license, and current car insurance.
