Provide in-home and community-based training and support for consumers, including assistance with daily living activities and behavioral interventions. Maintain accurate service documentation and transport consumers to community activities.
This is a full time position located at MLIH's Myyrtle, WV facility.
This position is essential to providing in home and/or community based consumer training and/or support.
1.
Assist the consumer, totally or partially, with toileting and dressing as needed.
2.
Participation in the development of the consumer’s Individual Program Plan (IPP).
3.
Maintain accurate written or electronic documentation of services delivered to the consumer and submit as directed in a timely manner, including a daily time line and service activity log.
4.
Implement behavioral intervention techniques to help reduce challenging behaviors.
5.
Must transport the consumer to community activities in personal vehicle.
6.
If the DSP provides Friends and Family transportation services for the consumer, then transportation vendor credentialing must be completed.
7.
Provide Human Resources with current driver’s license, automobile insurance policy and vehicle registration information.
8.
Able to provide respite at MLIH and/or in the community for the consumer.
9.
Perform other duties as may be assigned by immediate supervisor.
10.
Uphold the state and federal guidelines of the Waiver Program.
11.
Meet all defined performance measures as defined by MLIH.
Must be 18 years old, have a high school diploma or GED, and a valid driver’s license. This position requires the employee to be certified in CPR and First Aid on a two-year basis.
