Description: Per Diem Registered Nurse-Hospice Homecare for Schenectady, Saratoga and Montgomery Counties

The Community Hospice is seeking a compassionate and dedicated Per Diem Registered Nurse (RN) to join our Hospice Home Care team. This rewarding opportunity allows you to provide exceptional, patient-centered care while supporting patients and families in the comfort of their homes.

Availability needed for this per diem position would be during the week (Mon-Fri). Hours generally needed are 8a-4:30p. No weekends or holidays

What you will do

  • Coordinate and provide skilled nursing care to patients and families receiving hospice services at home.
  • Working as part of an interdisciplinary team, you will help develop and implement individualized care plans that promote comfort, dignity, and quality of life.

Responsibilities

  • Perform nursing assessments and provide skilled hospice care in the home setting.
  • Collaborate with patients, families, physicians, and interdisciplinary team members to develop comprehensive plans of care.
  • Monitor patient conditions and communicate changes to the care team.
  • Provide education, support, and guidance to patients and caregivers
  • Promote comfort-focused care and symptom management
  • Ensure timely and accurate clinical documentation.

What you will need

  • A current license to practice as a Registered Nurse in the State of New York
  • Associates Degree in Nursing, BSN preferred
  • The RN must be able to communicate effectively, verbally and written
  • 1 year experience as an RN required, preferably in Hospice/Home Care or acute care.
  • Valid Driver's License and Reliable vehicle
  • Strong communication, organizational, and clinical assessment skills.

No previous Hospice experience required. We welcome nurses who are interested in learning more about hospice care and are committed to providing compassionate, patient-centered care.

Per Diem Rates based on monthly hours commitment

Experienced Pay Range for non-per diem

$38.00-$50.18

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

Requirements

Requires a current New York State RN license, an Associate Degree in Nursing (BSN preferred), and at least one year of RN experience. A valid driver's license and reliable vehicle are mandatory for home visits.
