The Advanced Practice Nurse conducts patient visits and collaborates with an interdisciplinary team to develop and implement patient plans of care. They are also responsible for maintaining accurate medical records and assisting in the development of clinical practice standards.

Salary Range: $125,000 to $135,000 a year
1. Conduct visits to patients in accordance with local, state and federal regulations. 2. Communicate visit findings to designated members of the interdisciplinary team. 3. Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment). 4. Consult with other members of interdisciplinary team to contribute to the patient's plan of care. 5. Prepare and maintain accurate patient records, charts and documents. 6. Assist in development of clinical practice guidelines/standards in support of quality care as needed. 7. Ensure collaboration with patients, caregivers, medical directors and members of IDT.

Candidates must hold a Master's or Doctoral degree in Nursing and a valid Nurse Practitioner's license. At least one year of experience in Family, General, or Geriatric practice is required, along with a valid driver's license.
