Must be already certified as a Home Health Aide in NYS This is a 12 Hour Day Shift Position PACE CNY has been providing services since 1997 and is one of the only 123 PACE providers in 31 states.

The PACE CNY model is a Managed Long-Term Care program which provides an alternative to nursing home placement.
The goal is to maximize the independence of the elderly and enrich their quality of life by optimizing their functional ability.
This is achieved with the various services that are available to our PACE participants, including our very own PACE transportation system, 2 day centers that provide medical clinics and regularly scheduled primary care appointments, dining services, and social/recreational activities.
Top Five Responsibilities: Assist participants with Activities of Daily Living by providing direct personal care and homemaker services to participants at home and/or at the Day Center Promote optimal health by assisting in the development, provision and revision of a participants Plan of Care Assist with meals by serving and feeding participants and cleaning up after meal completion Monitor participants' health by noting changes in physical or emotional health and communicating this information to the Nurse and Interdisciplinary Team Follow Plan of Care by recording, monitoring and assisting (retrieving and opening container only) the participant with self-administered medications
