Collaborates with healthcare teams and families to manage prompt intake and admissions for the rehabilitation center. Conducts physical and psychological patient assessments to plan individualized care meeting IRF-PPS regulatory criteria.
Works collaboratively with all members of the health care team and the patient/family to ensure prompt and efficient intake and admissions for all patients referred to OLOL Rehabilitation Center. The Rehab Liaison conducts patient assessments that include physical and psychological factors in order to plan for individualized continuum of care needs and that meet regulatory criteria under the IRF-PPS.

Requires an Associate's Degree and three years of experience in general or specialty practice. Must hold a current unrestricted Louisiana license as an RN, LPN, or Therapy professional (PT/OT/ST/PTA).
