Help Individuals Successfully Transition Back into the Community The Hampton-Newport News Community Services Board (H-NNCSB) is seeking a compassionate and organized Discharge Planner to support adults who have been found Not Guilty by Reason of Insanity (NGRI) transitioning from Eastern State Hospital, Central State Hospital, or a local psychiatric hospital into community-based services.

In this role, you will coordinate services, connect individuals with community resources, and work closely with hospitals, families, and service providers to ensure a smooth transition from inpatient care to ongoing community support.

What You'll Do Coordinate discharge planning for adults returning to the community from Eastern State Hospital, Central State Hospital, and local psychiatric hospitals.

Meet with individuals to identify service needs and assist in developing plans for community supports.

Serve as the primary liaison between hospitals and the Community Services Board.

Connect individuals with mental health, housing, medical, financial, transportation, and other community resources.

Coordinate referrals and facilitate access to appropriate services.

Provide outreach and short-term follow-up to help ensure a successful transition after discharge.

Assist individuals in navigating available services and overcoming barriers to care.

Collaborate with families, hospitals, community providers, and internal staff to promote continuity of services.

Maintain accurate and timely documentation of services provided.

Advocate for individuals to ensure they receive the supports and resources needed to live successfully in the community.

Required Qualifications

Bachelor's Degree in Human Services Three (3) years of direct care experience in case management and counseling services to the assigned population
