About the role

The Clinical Pharmacist manages medication therapy for clinical programs including Anticoagulation, Diabetes, and MAT. Responsibilities include providing patient and provider education, performing therapeutic monitoring, and collaborating with healthcare teams to adjust medications and monitor labs.

SUMMARY

The Clinical Pharmacist will be instrumental in facilitating the safe and effective use of medication therapy at InterMed. Under the general direction of the Medical Director of Clinical Systems and in collaboration with physician and operational leaders, the Clinical Pharmacist will participate in the development, maintenance, and direction of InterMed’s clinical programs, including but not exclusively Anticoagulation, Diabetes and Medicated Assisted Therapy (MAT). Critical elements of this position include providing patient, staff, and provider education; performing therapeutic monitoring for efficacy and safety; evaluating for potential drug interaction; making recommendations regarding appropriate dosage adjustment; and effectively communicating to patients, families, and other members of the healthcare team.

  • Participate in interdepartmental clinical quality, value-based care and operational process improvement and cost savings opportunities, while ensuring compliance with federal and state regulations.
  • Remains current on departmental policies, procedures, formularies, and regulatory standards.
  • Demonstrates an understanding of the job description, performance expectations, and competency assessment.
  • Consultation for organization’s prescription refill protocol, making recommendations to the Quality Improvement Committee
  • Intervenes with appropriate health care professionals when necessary, and documents clinical interventions consistent with departmental policy.
  • Responsible for understanding and utilizing all technology.
  • Maintains courteous and effective interactions with colleagues and patients.

  • Pharmacists are responsible for their own professional development including, meeting all requirements for continuing education, keeping abreast of current professional literature, both from a clinical and medication safety perspective.
  • Pharmacists are expected to participate in select pharmacy department activities such as the clinical staff meeting and clinical conferences.
  • Performs all other duties as needed or directed by the Medical Director of Clinical Systems to meet the needs of the Primary Care departments.

  • Develop, implement, and continually update patient educational materials for clinical programs.
  • Work closely with all members of the healthcare team including but not exclusive to physicians, NPPAs, nurses, behavioral health specialists and others.
  • Develop, maintain, and engage in a collaborative practice model with Primary Care Physicians and NPPAs to initiate, adjust and discontinue medications, order, and monitor labs and refill prescriptions.
  • Provide adequate notes and documentation within the EHR or MTM platform to ensure proper reimbursement, patient safety, and to provide seamless management of patients amongst all staff.

  • Clinically reviews anticoagulation medication orders to identify patients receiving therapeutic doses of anticoagulation medications. Assessment should include appropriate drug selection, medication allergy, dose, route of administration, indication for anticoagulation, relevant laboratory results, therapeutic duplication, drug interactions and age of patient
  • Appropriately enters dosing recommendations and completes bridging calendars and other pertinent communications regarding the anticoagulation therapy into the Patient’s Medical Record.
  • Provides clinical guidance related to anticoagulation therapy to nurses, physicians, interns, medical students, and other members of the health care team.
  • Provides anticoagulation drug information to medical staff and patients upon request or when need is apparent. Documents request into the intervention database in accordance with departmental policy.
  • Responsible for day-to-day evaluation of patients on anticoagulation therapy to identify patients requiring close monitoring, evaluation of drug interactions, monitoring of therapeutic lab results and communicating recommended adjustments to therapy to the patient.
  • Serves as a liaison between the Anticoagulation Management Service and any health care provider.
  • Demonstrates competency in all practice areas in which Anticoagulation Clinical Pharmacy Specialist are performed. Has completed all required departmental competency requirements.
  • Provides Diabetic drug information to medical staff and patients upon request or when need is apparent. Documents request into the intervention database in accordance with departmental policy.
  • Participates in the Pharmacy Residency program as a clinical preceptor as applicable.
  • Maintains strict confidentiality in alignment with HIPAA (Health Insurance Portability and Accountability) guidelines and InterMed policies.

Knowledge, skills, and abilities

Education:

  • Must be a graduate of an accredited college of pharmacy with current Maine registration in Pharmacy.
  • License/Certifications:
  • Active unencumbered Maine pharmacist license
  • Board Certification in Ambulatory Care Pharmacy
  • Experience:
  • Strong baseline knowledge of Anticoagulant management.
  • Excellent communication skills with a customer-oriented approach.

Requirements

Candidates must be a graduate of an accredited college of pharmacy with an active, unencumbered Maine pharmacist license. Board Certification in Ambulatory Care Pharmacy and strong knowledge of anticoagulant management are required.
