• Range: $32.34 - $39.50 HourlyRegistered Nurse (RN) Full-Time/Part-Time | Grand Junction, CO Are you a compassionate Registered Nurse who thrives in a collaborative, patient-centered environment? PrimeHealth+ is looking for an RN to provide exceptional primary care nursing while working alongside providers and integrated care teams to improve the health of our community. What You'll Do Provide direct nursing care and clinical assessments for patients of all ages Triage patients in person and over the phone across multiple clinic sites Coordinate care for patients after hospital discharge to help prevent readmissions Collaborate with providers, medical assistants, and care coordination teams to deliver whole-person care Administer medications, perform medication reconciliation, and provide patient education Support quality initiatives, including vaccine programs, OSHA, CLIA, and cancer screening efforts Assist with co-visits, documentation, and care planning to enhance the patient experience What We're Looking For Bachelor's degree in Nursing (BSN) Current Colorado Registered Nurse (RN) license Strong clinical assessment, communication, and critical thinking skills Ability to multitask and thrive in a fast-paced, team-based environment Commitment to compassionate, evidence-based patient care QualificationsPreferred Qualifications One or more years of RN experience in a primary care or outpatient setting Bilingual in English and Spanish Why PrimeHealth+?
