About the role

Perform general nursing procedures, behavior management, and direct care for individuals with intellectual, developmental, and mental health needs. Responsibilities include administering medication, contributing to Individual Support Plans, and facilitating health education groups.

Satisfactory completion of post-employment testing is required to maintain employment.

Pay Ranges

$34.00 - $41.00/hr

*Sign-on bonus is for eligible applicants only after satisfactory evaluation. Must have a valid LVN license at time of hire. Bonus is not applicable for current employees/transfers.

Duties: and Responsibilities:

The Adult Skills Center (TASC) provides innovative and proactive services that empower individuals with diverse intellectual, developmental, and mental health needs to achieve their highest level of Independence and realize their greatest potential. The Licensed Vocational Nurse and Licensed Psychiatric Tech perform general nursing procedures, behavior management, direct care and training as dictated by the individuals' health and behavioral care needs.

Primary Duties

  • 1.
  • Assists with administration of medication policy to document and observe patient medication regimen.
  • 2.
  • Participates as a member of the interdisciplinary team and contributes to the development of the Individual Support Plan.
  • 3.
  • The LVN/LPT may function as lead staff on the shift and may oversee DSPs and/or CNAs.
  • 4.
  • Responsible for providing active programming at all times.
  • 5.
  • Assists with crisis management procedures as needed.
  • 6.
  • Provide support during client’s vocational training.
  • You may be required to work alongside client, engaged in the same job duties and adhering to same policies and procedures of the workplace (may include washing dishes, janitorial services, etc.).
  • 7.
  • Responsible for adherence to standards, procedures and schedules established by their supervisor.
  • 8.
  • Observes patients, charting and reporting changes in patients' conditions, such as adverse reactions to medication or treatment and taking any necessary action.
  • 9.
  • Evaluates medical concerns and directs patients to emergency medical care when required.
  • 10.
  • Works as part of a health care team to assess patient needs, plan and modify care and implement interventions.
  • 11.
  • Assists the RN in overseeing patients 12.
  • Facilitates educational groups on health and medication management.
  • 13.
  • Provides monitoring and supervision of patients to support non-clinical staff.
  • 14.
  • Transport clients in company vehicle/personal vehicle to and from program as well as to any scheduled activities throughout the day.
  • 15.
  • All other duties as assigned.

Qualifications

1. Possess and maintain a valid, active license from the State of California to practice as a Licensed Vocational Nurse or Licensed Psychiatric technician. 2. Must have current California Driver’s license/Insurance and clean DMV driving record.

  • 1.
  • Successful completion of pre-employment physical and drug screen, including current TB 2.
  • Successful completion of DOJ and FBI background check 3.
  • Successful completion of all trainings as required by applicable regulations, including, but not limited to PCM, DSP1 and DSP2 4.
  • The individual must be willing to work with a wide variety of symptoms related to mental health and developmental disabilities.
  • 5.
  • One year of experience working with persons with developmental disabilities required.
  • 6.
  • Maintain all certifications up to date 7.
  • Requires initiative, creativity and the ability to work independently with minimal supervision.

Physical Requirements

  • 1.
  • Ability to drive company and/or personal vehicle for work, including transporting clients to and from work and/or appointments as needed, maintain valid driver’s license and car insurance, if required to drive for position.
  • 2.
  • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
  • 3.
  • Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards 4.
  • While performing the duties of this job, the employee is frequently required to stand, walk, and sit.
  • The employee must occasionally lift and/or move up to 100 pounds.

The employee must provide physical assistance and/or push clients up to 250lbs in a wheelchair. Must be able to assist adult clients transitioning in and out of vehicle and in the restroom.

Completion of Professional Crisis Management Certification within 90 days of hire (provided by TASC) is required for this position and is not recommended for:

1) Individuals who have mobility and/or range of motion problems

2) Individuals with recent fractures or surgeries

3) Individuals who have suffered any kind of joint injury requiring medical attention in the past 6 months

4) Individuals with poor balance, flexibility or agility
