About the role

The social worker assesses patients' psychosocial-spiritual needs and develops comprehensive treatment and discharge plans. They collaborate with medical staff and outside agencies to coordinate continued care and represent patient interests in departmental meetings.

Summary

The social worker is responsible for assessing the patient’s psychosocial-spiritual needs, education and counseling.

Responsibilities

  • (20%)
  • Attends various medical staff or other departmental and committee meetings as a required in representing patient interests and/or concerns. (20%)*Administers patient assessment and writes evaluation report recommending treatment based on patient’s needs and function level. (20%)*Involve family in the treatment and discharge planning process if agreed upon by the patient. (20%)
  • Records patient progress notes in their respective charts. (20%)*Active in the patient discharge planning process and assists in the patient’s discharge to home or other extended care facility placement. OTHER DUTIES AND RESPONSIBILITIES: If Master Social Worker may provide supervision to Bachelors Social Worker staff as needed. Collaborates with outside agencies to coordinate continued care. Selects and schedules treatment activities consistent with the changing needs of the patient. Coordinates with other team members in the implementation of treatment plan; communicates recommendations to other disciplines. Will keep abreast of new regulations, procedures, equipment and methods. Evaluates patient goal and objective accomplishment and presents verbal and written criteria for termination of treatment. Knowledgeable of the Hospital’s computer system as it relates to the Social Services department and accessing patient related information. Excellent interpersonal and communication skills with the ability to deal with confidential information tactfully and diplomatically. Assist with the legal process as it relates to involuntary patients or guardianship. Must be able to use a personal computer, calculator, copy/fax machine, and other standard business office machines. Participates in Continuous Quality Improvement as required by the Medical Center and the job description. Understands and is accountable for the Medical Center's customer service program.

  • CONDITIONS: Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death. Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position. Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description. Overall vision and hearing is necessary with or without assisted device(s). Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching. Some exposure to blood borne pathogens and other potentially infectious material. Must follow MyMichigan Health bloodborne pathogen and TB testing as required. Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake. Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis.

Requirements

A Master of Social Work (MSW) is required, with a preference for two years of experience in a healthcare setting. Candidates must be working toward full licensure or hold a current license, and ACM certification is preferred.
