About the role

The LPN provides clinical nursing support in behavioral health settings, including medication administration and vital signs monitoring. They are responsible for maintaining compliant documentation and collaborating with a multidisciplinary team to implement treatment plans.

LA Health and Rehab Center Inc. (LHRC) is hiring a Licensed Practical Nurse (Contract) in Baton Rouge, LA to deliver steady, high-impact nursing support in behavioral health settings. This role exists to keep our care safe, organized, and moving forward—especially when patients need calm, consistent clinical follow-through.

  • Key responsibilities
  • Administer medications as ordered; track effects and report changes
  • Take and record vital signs and routine screenings; monitor physical/mental status
  • Support admission assessments and discharge preparation
  • Assist with treatment plan implementation and therapeutic interventions under supervision
  • Participate in de-escalation/crisis intervention to protect patient and staff safety
  • Provide patient/family education on medications, treatment, and coping strategies
  • Chart accurately and maintain compliant clinical documentation
  • Coordinate care with RNs, physicians, clinical director, psychiatrists, psychologists, and social workers
  • Required qualifications/skills
  • Graduate of an accredited nursing program; LPN license in good standing in Louisiana (or valid compact license)
  • Strong communication, empathy, resilience, and teamwork

Throughout the day you’ll connect with a multidisciplinary team, contributing updates that sharpen the overall approach and keep care aligned. You’ll wrap up by ensuring the story of the day is captured cleanly so the next handoff is seamless and the care journey keeps momentum.

Requirements

Candidates must be a graduate of an accredited nursing program and hold a valid LPN license in Louisiana or a compact license. Strong communication skills and resilience are required for handling crisis interventions and patient care.
