- 12-hour shifts
- Saturday and Sunday required

- Must be willing to travel within coverage area As a Home Health RN with Elara Caring, you provide one-on-one care in the patient’s home. - Manage and coordinate your own home health caseload
- Complete Start of Care, follow-up, and discharge visits
- Perform comprehensive patient assessments
- Develop and update individualized plans of care
- Complete OASIS documentation accurately and timely
- Educate patients and families on medications and disease management
- Monitor patient progress and report changes to physicians
- Collaborate with interdisciplinary care team members
- Ensure documentation meets compliance and regulatory standards ---------------------------------------------------------------------------------------------------------------------------- WHAT YOU NEED
- Active, unrestricted RN license
- At least 1 year of nursing experience
- Willingness to travel within the coverage area
- Reliable vehicle, valid driver’s license, and auto insurance
- Home Health experience preferred
