About the role

The Hospice RN Case Manager provides comprehensive health care and symptom management to terminal patients in various home and facility settings. They act as patient advocates, coordinating holistic care plans to improve comfort and quality of life for patients and their families.

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life.

Our service allows our team members to focus on one patient at a time providing care where the patient calls home.

While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals.

If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

Position Summary

The Hospice RN Case Manager delivers compassionate and comprehensive health care services to Hospice patients in a home care setting, or wherever the patient calls home. Home care settings may consist of a private residence, skilled nursing facility, personal care home, and/or other interim living arrangements. The Hospice RN Case Manager’s goal for excellent patient care standards begins with active listening to create and implement a holistic plan of care based on the patient’s goals and preferences. The Hospice RN Case Manager serves as the patient advocate to support, guide, and coordinate care for patients, families, and caregivers as they navigate Hospice Care. The plan of care will include managing symptoms through pharmacologic and non-pharmacologic interventions for optimal comfort, functionality, and safety. Frequent monitoring and follow-up are required to evaluate the efficacy of treatments, plans of care, and to offer emotional support to both the patient and the caregiver. A Registered Nurse in this position will have strong communication skills, the competence to work autonomously, professionalism to work within an interdisciplinary team, and the ability to provide care with empathy. This position offers ongoing opportunities to make a significant and rewarding difference in patients’ lives during a challenging time. Position Qualifications:

Graduate of an accredited school of registered nursing.

Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or through a participating Compact State Board of Nursing.

Minimum of one (1) year of general nursing experience preferred; new graduate nurses will be considered.

Demonstrates strong clinical nursing skills and the ability to maintain professionalism during emotionally challenging situations.

Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.

Adheres to accepted professional, ethical, and legal standards of nursing practice.

Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying.

Maintains current CPR certification.

Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements. Requirements

Candidates must be a graduate of an accredited nursing school and hold a current RN license in Kentucky or a Compact State. A current CPR certification and a valid driver's license with a reliable vehicle are also required.
