About the role

The Occupational Therapist conducts comprehensive evaluations and implements individualized treatment plans to improve patients' independence in daily activities. They collaborate with an interdisciplinary team and educate patients and caregivers on adaptive strategies and equipment.

Job Title: Occupational Therapist (Home Health)

Job Summary

We are seeking a dedicated and compassionate Occupational Therapist to provide in-home therapy services to our patients. In this role, you will evaluate patients, develop individualized care plans, and deliver therapeutic interventions to improve independence and overall quality of life. You will collaborate closely with physicians, caregivers, and the interdisciplinary care team to ensure optimal patient outcomes.

Key Responsibilities

  • Conduct comprehensive occupational therapy evaluations to assess patients’ physical, cognitive, and functional abilities.
  • Develop and implement individualized treatment plans with short- and long-term goals, in collaboration with the attending physician.
  • Provide hands-on therapy to improve patients’ independence in daily living activities.
  • Educate patients and family members on therapeutic techniques, adaptive strategies, and self-care activities.
  • Train patients and caregivers in the safe use of orthotic/prosthetic devices and adaptive equipment; may design or modify assistive devices as needed.
  • Perform ongoing assessments and re-evaluations to monitor progress and adjust treatment plans accordingly.
  • Maintain accurate and timely clinical documentation in accordance with agency policies and regulatory standards.
  • Communicate regularly with physicians regarding patient progress and recommend modifications to care plans as appropriate.
  • Collaborate with interdisciplinary team members, including nurses, physical therapists, and case managers.
  • Participate in case conferences, team meetings, and required in-service trainings.
  • Coordinate outpatient occupational therapy services when care cannot be fully provided in the home.
  • Provide guidance and training to other staff members on therapy techniques, equipment, and patient care strategies as needed.

Physical Requirements

  • Ability to safely assist and transfer patients using proper body mechanics.
  • Ability to perform physical tasks such as standing, walking, bending, lifting, and assisting with therapeutic exercises.

Qualifications

  • Bachelor’s or Master’s degree in Occupational Therapy from an accredited program.
  • Current Occupational Therapist license in the State of Michigan.
  • CPR certification preferred.
  • Valid driver’s license, reliable transportation, and proof of auto insurance.
  • Strong communication, organizational, and interpersonal skills.
