About the role

Support the Director of Admissions in welcoming and admitting new residents to a long-term care and rehabilitation center. Complete essential documentation including psychosocial assessments, care plans, and discharge plans to ensure a positive patient experience.

Do you have experience with Admissions, patient registration, care coordination, discharge planning, or other related healthcare office experience and looking for a great new opportunity? Then come join our Team!

As a Social Services Admissions assistant, you will support the Director of Admissions function in a long-term care and short-term rehabilitation nursing center. You will work closely with the Director of Admissions Manager to welcome and admit new residents and patients, complete documents related psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans. You'll enjoy a Monday – Friday, dayshift work schedule.

Qualifications

  • Minimum high school diploma or equivalent required.
  • Experience with insurance authorizations and Admissions
  • One year supervised social work experience working directly with individuals in a health care setting.
  • One year supervised social work experience working directly with individuals in a health care setting.
  • One or more years’ experience in health care education/ marketing / insurance or commensurate educations preferred.
  • Must be familiar with medical terminology.
  • Proficient in the use of personal computer.
