Perform comprehensive initial and ongoing assessments and provide therapeutic interventions for patients in their homes. Coordinate care with the interdisciplinary healthcare team and report patient conditions to the Rehab Director.
Greeley, Loveland, Fort Collins and surrounding areas.
Pay
Rates: $75-$150 per visit, (depending on visit type), PLUS mileage and travel time compensation!
Status: Part-Time, minimum of 5 visits per week.
