Key

Responsibilities: Conduct initial and ongoing assessments of patient needs. Implement and adjust individualized care plans. Ensure compliance with hospice policies and Medicare regulations. Educate patients and families on self-care and end-of-life options. Supervise paraprofessionals delivering patient care. Document and report clinical findings and participate in interdisciplinary meetings. Enhance patient satisfaction and experience.

Qualifications

Current Michigan LPN License in good standing. Minimum of 1 year experience in hospice or palliative care. Strong empathy and communication skills. Flexibility to respond to varying patient needs.
