The Case Manager develops supportive relationships with clients; link them to community- based medical and mental health care, and provide crisis intervention as needed. The Case Manager provides support with accessing primary medical care and other necessary services to increase clients’ capacity for independent living and monitors progress, providing referrals as needed. The Case Manager completes required documentation in a timely manner and participates in case conferences, on-site and community-based meetings and trainings as required. • Essential duties: Completing comprehensive assessments, service plans, progress notes Monitoring and reassess clients’ needs for referrals and services Provide psychoeducation on relevant topics like substance use disorders, entitlements, etc. Provides individual and group workshops Work collaboratively in an inter-disciplinary team to ensure comprehensive, client-centered services are delivered and documented.
