Provide comprehensive case management and strengths-based assessments for youth with complex behavioral health needs and their families. Coordinate psychiatric services, facilitate family team meetings, and provide 24-hour crisis coverage to maintain youth in the least restrictive settings.
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Work primarily with youth with co-occurring intellectual disorder, serious mental illness or emotional disorders, Autism Spectrum Disorder and their families, with the goal to maintain the youth in the least restrictive setting with regards to home, school and community programs. Physical demands: Minimal in nature
Bachelor’s degree in human services field from an accredited college/university. One to two years’ experience in the behavioral health field and experience working with children and adolescents preferred. Driver's License and Vehicle Required.
