About the role

Provide clinical and residential support to individuals, including case management and assistance with daily living skills. Coordinate with multidisciplinary treatment teams and maintain accurate service documentation for Medicaid billing.

Position overview

Under the direct supervision of the Team Leader, the Social Rehab Counselor (SRC) provides both clinical and residential support to individuals served. Responsibilities include community support, case management, daily living assistance, and promoting a safe, supportive, and goal-oriented home environment.

Salary: $44,075

Hours: Sunday-Thursday 11 pm-7 am

Location: Naperville, IL

Reports to

Team Leader

Network or department: Network 4

Essential functions

Provide community support, including:

  • Transportation to day programs, medical, benefits, or employment appointments
  • Participation in leisure and goal-related activities
  • Assistance with grocery shopping and errands
  • Deliver case management services such as:
  • Support with medical benefits
  • Coordination with therapists and treatment teams
  • Support individuals with:
  • Medication management
  • Daily living skills (meal preparation, laundry, home cleanliness)
  • Maintain accurate and timely service documentation and Medicaid billing
  • Participate in house, staff, and mandatory monthly MI supervision meetings
  • Collaborate with multidisciplinary treatment teams, including PSR and Supported Employment staff
  • Actively participate in case staffing as needed
  • Communicate any concerns or issues to the Team Leader promptly
  • Meet or exceed established service billing benchmarks

  • Ability to collaborate with diverse stakeholders including staff, families, and community agencies
  • Capable of planning, organizing, and executing program goals and procedures
  • Self-motivated and able to work independently
  • Professional demeanor that supports a therapeutic environment

Physical demands

  • Lift over 40 pounds from floor to waist unassisted
  • Push and pull up to 75 pounds; push wheelchairs up to 200 pounds
  • Perform physical movements such as squatting, stooping, crouching, and kneeling
  • Stand, sit, or remain stationary for prolonged periods
  • Drive and operate motor vehicles
  • Engage in repetitive motions using hands and fingers for computer and phone use

Qualifications

• Bachelor’s Degree in Social Work, Psychology, or related field
