About the role

Screen patients for social determinants of health and assist them in accessing community resources, insurance, and financial support. Provide intensive case management under the ECM model and deliver prenatal education services to pregnant women.

Bilingual (English/Spanish)

Committed to providing high-quality services to underserved patients in Orange County.

Willingness to work as part of a collaborative team that includes, social worker, physicians, nurse practitioners, and psychiatrist.

Ambulatory (outpatient) care

Part-Time - 16 hours per week.

Our health centers use a team-based model of care that includes family medicine, obstetrics, women’s health, oral and vision care, preventative and educational health, mental health, patient advocacy, case management, and community health.

Most of our patients live below 200% of the Federal Poverty level and are best served in Spanish.

Essential Functions (What You Do) Screens patients for social determinants of health and partner with the patient and other care team members to strategize mitigation of the social determinants impacting the patient health.

Assists patients with the completion for forms for available support systems, including applications for disability, rental assistance, SNAP, housing vouchers and temporary cash assistance.

Assists patients with questions and issues regarding Cal-Optima benefits, including transportation, DMEs, etc.

Collaborates with patient advocates to ensure patients obtain health insurance coverage when coverage is available.

Collaborate with providers to directly provide patients with information regarding their health and how best to manage their health to achieve their goals and/or connect patients to available health education resources.

Informs patients on how to use available community resources and benefits.

Provides Prenatal Education Services to pregnant women under the Comprehensive Perinatal Services (CPSP) Responsible for accurate, complete and timely documentation of all patient contacts.

Maintains and manages an active caseload of patients, maintaining productivity measures for department initiatives.

Under the ECM (Enhanced Care Management) model, works proactively and collaboratively to provide intensive case management to patients with complex care needs and/or at high-risk and/or non-compliant.

Using motivational interviewing, coaching, SBIRT, DME, care coordination and other appropriate tools, directly interact with patients to establish goals and develop plans with the patient on how they will achieve their goals.

Meets all outcome measures for case management and caseload.

Documents all ECM patients, submitting reports in accordance with all required timelines.

Pay Range: $23.87 to $28.30 per hour

Requirements

Education: Bachelor’s Degree in social sciences (health sciences, social work, human services), or related program

License/ Certifications: N/A

Experience

Minimum of 12 months’ experience in case management or equivalent education/ training experience.

Knowledge/Skills/Abilities

Able to communicate effectively to all levels of patients, helping them traverse social programs and social services with comfort and accessibility. Demonstrates the ability to read, write and speak English and Spanish without an interpreter. Demonstrates interpersonal/cultural sensitivity and work with a diverse and often underserved patient population. Exercises discretion and professionalism in handling patient issues with sensitivity and respect. Effective written and verbal communication across all levels of the organization and with community partners. Highly organized with strong time management skills, being able to perform simultaneous tasks and prioritize tasks effectively and follow through in a timely manner. Proficiency with EMR systems.
