About the role

The Care Coordinator works with a multi-disciplinary team to deliver comprehensive services for high-risk pediatric and chronic illness patients. They act as the primary family contact to implement tailored healthcare plans and reduce diabetes-related complications.

The Family Medicine Care Coordinator works within a multi-disciplinary health care team in the delivery of comprehensive services for patients diagnosed with chronic illness and high risk pediatric patients in a primary care setting. Acting as the primary contact person for the family, the Family Medicine Care Coordinator assists the health care team in the development and implementation of a health care plan tailored to the needs of the client and the client's family in order to promote continuity of care, improves referral tracking, and ultimately reduces the rate and severity of diabetes-related complications.

Program Manager II - Chronic Disease, Family Medicine RN Supervisor at Health Center

Qualifications

1. Must have a Bachelor's degree in Health Education/Health related field or hold California state licensure for vocational nursing (LVN). While this position is considered an entry-level position, experience in practical nursing, preferably in an ambulatory care facility or a health education background in chronic disease is strongly desirable. 2. Bilingual in English and Spanish required, with excellent written and oral communication skills. 3. Possess the ability to work independently and as team member. 4. Cultural competency required. 5. Possess excellent organizational and problem solving skills and have the ability to prioritize. 6. Be willing to travel to provide outreach presentations. Have current CA Driver's License, and available vehicle, and valid liability insurance. 7. Experience with basic computer skills including word processing and data entry. 8. Process the ability to perform a variety of tasks after training. Requirements

Candidates must have a Bachelor's degree in a health-related field or a California LVN license and be bilingual in English and Spanish. A valid CA driver's license and vehicle are required for outreach travel.
