What You’ll Do As a Home Care Physical Therapist, you’ll work directly with patients in their homes and communities to help them regain mobility, improve function, and maintain independence. Responsibilities include: Evaluate patients and develop individualized treatment plans Deliver skilled physical therapy services in the home setting Build meaningful patient relationships through one-on-one care Complete documentation accurately and on time Coordinate schedules and communicate with the clinical support team Drive clinical initiatives focused on delivering exceptional patient outcomes
