Position Summary

The Nurse LPN - ACT Program supports our Assertive Community Treatment team by delivering safe, timely nursing services for adults with severe and persistent mental illness. This role is essential to maintaining medication continuity, clinical stability, and accurate documentation in a community-focused model of care.

Key responsibilities include psychiatric nursing assessments, medication administration (including narcotic dispensing as applicable), therapeutic support, crisis response assistance, and maintaining complete medical records in accordance with regulatory and program standards.

Required qualifications/skills

Current Licensed Practical Nurse (LPN) license with the Virginia State Board of Nursing Graduate of an accredited LPN school of nursing with one (1) year of medication administration experience in substance abuse treatment, psychiatric, or long-term care settings, including dispensing of narcotic medication Valid driver’s license for community-based client support Strong communication skills and ability to work effectively on a multidisciplinary team Our Organization Since 1971, the Hampton-Newport News Community Services Board has provided the highest quality of behavioral health services to residents of the cities of Hampton and Newport News, Virginia, offering a comprehensive array of programs for both children and adults living with mental illness, developmental disabilities, and/or substance use disorders.

Your Typical Day Your day runs Monday through Friday, 8

  • 00 a.m. to 5:00 p.m., anchored in an onsite ACT setting with planned touchpoints and real-time coordination. You’ll start by reviewing updates, prioritizing client needs, and aligning the plan with team members. Throughout the day, you’ll balance scheduled client interactions with walk-in needs, adjusting calmly when symptoms escalate or a situation becomes urgent. You’ll spend focused time ensuring records are current and complete, then connect with colleagues to confirm next steps and continuity of care. The day closes with clear handoffs, documentation checks, and preparation for the next day’s caseload. Our Benefits

Requirements
