Provide hands-on personal care including bathing, grooming, dressing, and hygiene assistance Monitor and record patient vitals, health changes, and daily activities Assist with mobility, transfers, and ambulation to ensure patient safety Administer medication reminders and follow individualized care plans Prepare nutritious meals and assist with feeding as needed Offer companionship and emotional support to elderly clients and their families Maintain a clean, safe, and comfortable home environment for clients Communicate effectively with supervisors and healthcare team members regarding patient status
